In order to receive your Certificate of Completion, please click on the class(es) you attended and fill out the brief survey for each class. Be sure to fill out the form with your name as you’d like it to appear on your certificate. Upon submitting the survey, you will be emailed a PDF version of your certificate.
If you need an OPI Renewal Units certificate, please go ahead and fill out this form, so we receive your feedback about the classes you attended.
If you do not know your license number, you may enter zeros (00000) or numbers (12345).
